You should always keep a copy of your contact lens prescription, especially if you are traveling or going on vacation. The best way to get your prescription is to ask your doctor for a copy for business travel and vacations.

If your doctor's office insists on a written release form, follow the 3 steps below.

1. Print out our Contact Lens Release Form
2. Fill out the top half of the form, then fax, mail or call it in to your doctor's office.
3. Once the prescription information is complete, fax or mail us a copy.

To expedite your order, always provide your eye doctor's name and telephone number on the secure checkout form. Our doctors will call to verify your prescription and insure prompt delivery of your order.
